These forms will be required and fees must be paid to register with the      MidState Stars Soccer Club.

At this time we are only accepting the ages of 14 - 18 year olds, boys and girls for our U18 coed team. We hope in the future, as our club grows, that teams for U10, U12, & U14 can be provided. We also plan to have separate gender teams when the number of our memberships' allow.
